How does The Silent Patient end?

The novel's central mystery — why the celebrated painter Alicia Berenson shot her husband Gabriel five times in the face and then never spoke again — is unraveled through the alternating narration of her psychotherapist, Theo Faber, and passages from Alicia's own diary. Theo, who has engineered his way into becoming Alicia's therapist at the Grove, slowly gets her to open up, using art materials and patient persistence to draw her out of six years of silence. The twist is that Theo himself is responsible for setting the murder in motion. He discovers that Kathy has been having an affair with Gabriel, Alicia's husband. Consumed by obsession and rage, Theo stalks Gabriel, then dons a mask and breaks into the Berenson home to confront Alicia and reveal her husband's infidelity to her directly. The shock of the masked intrusion, combined with the devastating revelation about Gabriel's affair, triggers a psychotic break in Alicia. That same night, in her disordered state, she takes Gabriel's gun and shoots him five times. Overwhelmed by what she has done, she then attempts suicide and, unable to face or explain the killing, resolves never to speak again.

After Theo finally coaxes Alicia's story out of her, she recounts the night of the murder, but Theo and the Grove's staff dismiss her account as delusion. Shortly afterward, Alicia is found unconscious, having been injected with a near-lethal dose of painkillers. A police investigation into the incident turns up Alicia's diary, in which she had recorded the events of that night, including the masked stranger's intrusion. The diary makes clear that Theo was that intruder, and implicates him both in precipitating Gabriel's murder and in silencing Alicia once she began to remember and speak the truth.

The book closes abruptly and without a tidy resolution: an investigator arrives at Theo's own home carrying Alicia's diary and begins reading aloud from her final entry, the one that names Theo as the masked man from that night. The narrative ends there, on the verge of Theo's exposure, leaving his ultimate fate — arrest, confession, or otherwise — outside the frame of the story.
